What makes a support record audit-grade
An audit-grade record is one an auditor can trace end to end. Read it forward as everyday practice, or backward from evidenced impact to the original assessed need. The platform carries the traceability, so the admin stays light.
Assessed need
A cognitive assessment surfaces a domain scored 84 or below, a potential barrier. It is a signal for tutor judgement, never a diagnosis.
Matched reasonable adjustment
The barrier is matched to a reasonable adjustment that carries its Intent: why this support, for this learner.
Support plan
The adjustment flows into the learner's support plan, so intent becomes a plan to deliver.
Delivery
The tutor delivers the support and confirms it: the meeting, its type, date and hours.
Monthly record
Each month the support is recorded against Intent, Implementation and Impact (the Three I's): what was done, and what changed.

The mechanism differs by provider, and cost recovery is always your decision. For apprenticeship and skills providers, good records support a defensible learning support funding position: you decide eligibility, code it and file via the Individualised Learner Record (ILR), and Cognassist supplies the evidence (the Intent, Implementation and Impact data, the Above and Beyond classification, and the meeting type, date and hours). For further education (FE) colleges, the Inclusive Mainstream Fund (£83 million a year for learners aged 16 to 19, allocated on low prior attainment, automatic from around July 2026) pays you to build and evidence the inclusive core offer, set out in an accountability statement. For higher education, Cognassist is the inclusive-infrastructure layer you run before the Disabled Students' Allowance. The funding hub has the mechanism for your provider type.
